To my valued patients,

The time has finally come to announce I am retiring from active practice, effective December 31, 2017. Some of you have been me since Portland to Salem/Keizer and Wilsonville. I have valued our long relationship. I hope I have faithfully cared for you and added to the quality of your lives as you have done for me; as being a Physician is truly a two-way street.

I am happy to announce that The Dermatology Clinic will be taking over my practice in Keizer. Joel James PA-C will be staying at the Keizer office. He has been with me for over 10 years and I know him to be a knowledgeable and caring provider. I trust him to continue excellent Dermatologic Care. In addition Erik Kokkonen, M.D., Preston Chadwick, M.D., and Shashi Srinivasan, M.D. will be at the Keizer office as well as at the clinic on Grear Street and at Main Street in Wilsonville and there will be an addition of Dr. Reyna Swift in the fall of 2018, after she completes her training at USC.

It is important for you to continue your skin cancer surveillance on schedule, but particularly important if you have had a Melanoma or Merkel Cell Carcinoma, or multiple Basal Cell Carcinomas, or a Squamous Cell Carcinoma on the head or neck, or if you have a decreased immune system, or have had light therapy in my office, or have a history of excessive sun exposure and sun burns. You realize that means most of you!

Your medical records are confidential. Your active medical record will be integrated into the medical records of The Dermatology Clinic . Your records, including photographs, prior to 2015 will continue to be available but housed within an electronic vault. Please contact me or The Dermatology Clinic if you choose to have your medical file and/or photographs mailed to you.

I hope I have supplied the information you will need to allow you to continue in good skin health. If not, you know where I am until December 27th,2017. After that I will still be around continuing the Tattoo removal program at MacLaren for the Oregon Youth Authority, spending time with my Grandchildren, doing a few small research projects with OHSU and some travel; Oh yes, there still is the garage to clean out and time to read and sleep in.

I have truly valued our relationship and thank you for your loyalty and friendship over the years, I will miss you all! Best wishes for your future and wear your sun screen; SPF 30 or greater and remember to reapply!

I will continue to use my email address if you wish to contact me.

Carolyn I. Hale, M.D.
